COMMUNITY SUPPORT STAFF

Reporting to the Community Support Supervisor, the Community Support position is responsible for integrating individuals with disabilities into the community to participate in work, recreational, leisure, employment, and volunteer activities. The Community Support Staff will provide training, support needs and direct care as well as help achieve goals based on the individual’s preferences and abilities.

RESPONSIILITIES

  • Demonstrate a person-centered practice and supports each person’s choice in the activities, locations, and people with whom that person interacts on a daily basis.

  • Observe individuals’ behaviors and changes in physical condition, utilize prescribed strategies to assist the individual, and document and report any incident accordingly.

  • Assist and encourage individuals, without being directive, to engage in various activities in the home and community that are of interest and meaningful to the individual.

  • Understand and implement/follow current ISPs of each person being supported and develop strategies that lead to the achievement of individuals’ personal outcomes.

  • Participate in individual planning processes to define a community building focus for people supported and ensure the plan is meaningful and beneficial to the person.

  • Provide assistance to individuals with intellectual disabilities in all areas of daily living so that these individuals can live and work in their community to foster maximum independence of individuals. These services include but are not limited to habilitation, health needs, personal care and hygiene, employment, transportation, recreation, housekeeping and other home management-related supports.

  • Ensures at all times that the health and safety of each person supported is maintained. Apply emergency procedures as necessary.

  • Immediately reports any incidents in accordance with regulatory and licensing requirements.

  • Provide transportation to different community activities/locations based upon the person’s needs and interest in a safe manner.

  • Participate in training, staff meetings, and supervision meetings as scheduled.

  • Complete and maintain required records and documentation, such as progress notes, attendance, and work logs.

  • Adhere to HIPPA Guidelines.

  • Follow Allegiant Human Services Policies and Procedures.

Job Types: Full-time, Part-time

Salary: $14.00 - $16.00 / hour
